Amber Jacobs, Nurse Practitioner, is now offering primary care and same-day sick visits at Vitamin Sea Infusions.

Whether you need help with an acute illness, medication management, chronic condition follow-up, or general wellness concerns, Amber provides personalized care with a holistic approach focused on finding and treating the root cause.

Appointment Details:

• Up to two concerns per visit

• Visits are scheduled for up to 30 minutes

• Additional follow-up appointments may be recommended for diagnostic review, treatment planning, or more complex concerns

Sports Physicals-

Prepare for the upcoming sports season with a comprehensive sports physical at our clinic. During your appointment, our provider will complete a thorough medical evaluation, including a review of your medical history, vital signs, and a physical examination. We also perform a basic vision screening and can complete an EKG when medically indicated or required.
